Another escalation in the fight against diabetes and obesity- A "Silver Bullet" in the making?

There are three hormones in human system that control the metabolism and absorption of glucose and they are now the focus of attention in developing appropriate therapy to attack the diabetes and obesity syndromes widely considered to be reaching epidemic proportions in many countries. These are glucagon, glucagon like peptide (GLP) and gastric inhibitory polypeptide (GIP) working in different ways in different locations in the body. In a remarkable breakthrough scientists have now achieved integration of these three hormones in a single molecule to derive all the benefits from them. While glucagon balances any hypoglycemia condition by releasing glucose from glycogen in the liver, GLP inhibits gastric secretion delaying carbohydrate absorption creating a satiety effect. GIP has a major role in neutralizing stomach acid besides slowing down transfer of food across the intestine. GIP also plays a role in insulin secretion enabling the body to metabolize glucose faster and its influence on lipid metabolism is recognized for its potential use against obesity. By combining these three hormones the researchers seem to have succeeded in creating a "silver bullet"  that can deal with both diabetes and obesity. Here is a gist of these findings which appears to be really path breaking in its impact.

"In 2012, we covered work led by Professor Richard DiMarchi that showed linking two hormones into a single molecule held promise as a treatment for obesity. DiMarchi followed this up last year by combining the properties of two endocrine hormones to provide an effective treatment for both obesity and adult-onset diabetes. Continuing in this vein, DiMarchi has now co-led a study whereby obesity and diabetes were effectively cured in lab animals by adding a third hormone to the molecular mix. Clinical work carried out last year, which included human clinical trials, showed a peptide combining the properties of two endocrine hormones, GLP-1 (glucagon-like peptide 1) and GIP (gastric inhibitory polypeptide), provided an effective treatment for adult-onset diabetes. Now the team, co-led by DiMarchi, the Indiana University (IU) Distinguished Professor of Chemistry, and Matthias Tschöp, director of the Institute for Diabetes and Obesity at the German Research Center for Environmental Health, has developed a new molecule that includes a third hormone, glucagon. GLP-1 and GIP are involved in enhancing insulin action and reducing blood glucose in the body, with GLP-1 also acting to reduce appetite. Meanwhile, glucagon is involved in improving liver function and increasing the long-term rate at which calories are burned. Molecularly combining these three hormones forms what is known as a triple agonist, which can bind to and activate receptors in the body and produce certain biological responses. "This peptide represents the first rationally designed, fully potent and balanced triple agonist ever achieved in the treatment of any disease," says DiMarch. "The benefits of the previously reported individual co-agonists have been integrated to a single molecule of triple action that provides unprecedented efficacy to lower body weight and control metabolism." The researchers say that in preclinical trials, this new peptide lowered blood sugar levels and reduced body fat better than all existing drugs. By triggering improved glucose sensitivity, reducing appetite and enhancing calorie burning, it was able to reduce body weight in rodents by around 30 percent, (nearly twice as much as the preceding GLP-1/GIP double hormone), and essentially cured the animals of obesity, diabetes and associated lipid abnormalities. "This triple hormone effect in a single molecule shows results never achieved before,"said co-first author Brian Finan, a scientist at the Helmholtz Diabetes Center. "A number of metabolic control centers are influenced simultaneously, namely in the pancreas, liver, fat depots and brain."

According to the innovators of this novel 3-in-1 hormone peptide molecule, extensive animal studies and limited clinical trials have more or less confirmed about its efficacy in increasing glucose sensitivity, reducing appetite significantly and enhancing burning of calories dramatically. Reduction of body weight to the extent of 30% is a remarkable achievement which will gladden the hearts of millions of people with BMI beyond 30. Hopefully this hormone combination will become the standard bearer in the coming years for world wide use in bringing down obesity, curing diabetes and correcting many lipid abnormalities for which there are no satisfactory treatment protocols at present. Though lot has been said about the efficiency and effectiveness of the new peptide molecule, nothing is mentioned as to how it is produced or how stable it is when administered in human beings. Since only GIP works at the intestinal level, how effectively the peptide is absorbed in the blood is also not clear. As human clinical trials have been carried out one can assume that the claims made by the innovators are valid. Whether there will be any other side reactions in some people is also not known. Let us hope we will have one of the most effective medicines to overcome the twin curses in the form of diabetes and obesity soon and association of one of the giants of pharma industry with this study further reinforces our hope.

"FOIE GRAS"-MAN'S CRUELTY AGAINST ANIMALS

Modern society is much more sensitive to the sufferings of animals which are used for producing a number of foods for omnivores and carnivores whose population outnumber herbivores by almost 9 to 1. Whether it is breeding, raising, handling or butchering, well established universal guidelines are followed to be as humane as possible. It is another matter that many practitioners in the animal food production sector get away with frequent violations of these standards though humane society campaigning for a better deal to animals always keeps a watch over the industry. One is impressed by the sustained campaigns to change the highly congested cage system in the poultry industry to provide more space to the birds while free grazing cows are encouraged in the beef industry. A not commonly known food from Geese and Duck known as 'Foie Gras", most popular in France is made by fattening the liver of these birds, generally through force feeding  and is considered a delicacy by many food connoisseurs. There is a strong movement which opposes continued practice of this cruel practice by pleading for its ban globally. Many countries like Israel, Turkey and some in Europe have already banned its production and sale and the latest to join this group is the State of California in the US which has promulgated law for punishing those indulging in production or sale of Foie Gras beginning July 1 this year. Here is more on this development which has made news recently.       

"This is the last week for legal foie gras in California. A law to shut down the making, cooking and selling of super-fatty goose or duck liver takes effect July 1. Chow hounds from Chico to Chula Vista have been opening their wallets for restaurants' foie gras menus, gorging themselves like birds before the winter migration. The law's purpose is to end gavage, the ancient practice of forcing grain down the throat of a goose or duck until its liver is grossly enlarged. The law's critics say gavage is hardly more stress-inducing than the many other things humans do to the animals they eat. The corporate operations that grow and slaughter pigs, poultry and cattle represent animal cruelty on an immense scale, they say, about which the foie gras ban does nothing. But the law's supporters argue that even small steps toward humaneness are important, and point to several countries in Europe that have banned foie gras. It is not known how energetically the state will enforce the ban and if any restaurant chefs will be willing to risk a $1,000 fine. Chicago enacted a foie gras ban in 2006. It was widely mocked and flouted and, after a couple of years, repealed. Chefs in California are already pushing for a repeal bill. Meanwhile, "faux gras" recipes are proliferating on the Web. One chef suggests soaking chicken livers overnight in milk with garlic, thyme, salt and pepper, searing them briefly, then puréeing them in a food processor with half their weight in soft butter. It's your basic chicken-liver mousse, not foie gras but good. "You could mix almost anything with half its weight in butter and have a very nice spread," said Mark Bittman, one of The Times's experts on such things".

Out of about 25000 tons of Foie Gras produced annually more than 70% is in France whose population love to have this specialty product during Christmas season. It appears many reports regarding wide scale prevalence of forced feeding seem to be misleading because birds like Geese and Ducks have a tendency to feed more than what they really need and if suitably designed feed is provided ad libitum they will develop fatty liver in no time obviating the need for forced feeding. Some of the producers claim that they do not cause any discomfort to the bird and replaced the metal tubing for force feeding with softer ones. A relevant question is why the product itself is banned if there are alternative methods available to make it and why such ban is not confined to the production technique deployed. After all Foie Gras has a 2500 years of history behind it and it is closely linked to the French culture. As a measure to discourage meat consumption it may be a welcome move, as such bans remove one particular meat product from the dining tables of at least a few families.

ALCOHOL DRINKS-NEED FOR MANDATORY LABELING

The compulsory labeling of any thing that goes into the body has been very effective as a means of consumer education and conveying the nature of the contents in an unambiguous manner. Different countries follow different rules but packed food materials always have information negative as well as positive which enable the consumers to make a correct decision in choosing the right products from the retail shelves. It is another matter that lot of unsubstantiated health claims are made on such labels by some manufacturers. Normally all foods made in the organized sector of food industry can be expected to be safe and of good quality, conforming to the specifications laid down in the statute book. While food and medicines are covered under statutory labeling rules, alcoholic beverages belong to a group that is not required to conform to such rules but demands are emerging to cover these products also. Few countries have introduced such labeling regulations though most such practices are voluntary, agreed upon by the industry on its own volition. Considering that alcohol is more a drug than a beverage, the declaration must be clear regarding the negative consequences of consuming alcohol. Besides its debilitating impact on liver, alcohol is also considered a human carcinogen deserving utmost caution by the consumers. Australia is currently debating on this issue and in what form the ultimate decision will come is not certain as of now.

In Australia, as in many other countries, alcohol is not labelled in the same way that food is. So it's exempt from the usual requirements for anything else you take into your body, which are required to have labels listing all ingredients and composition from a nutritional viewpoint. That's a peculiarity that has come out of history but it doesn't make sense because you are taking in calories or kilojoules into your body when you drink. In recent weeks, DrinkWise, which is an industry-funded social aspects organization, has come out with voluntary standards on labelling, which they hope 80% of the industry will follow. And yesterday the Alcohol Education and Rehabilitation Foundation (AERF)released a report calling for a Government-imposed health warning label system. Their proposed label is much more detailed and much more rigorous than what is currently being implemented by the industry under the DrinkWise initiative. The AERF recommends rotating five warning labels because one of the problems with warning labels is that you get very used to it after a while – as you do with anything else you see every day – and you don't notice them. If you change the message periodically and if there are a variety of messages being displayed, then people are much more likely to notice them and go on noticing them. The main precedents for this kind of labelling is on cigarette packaging but there is another example for alcohol warning labels in Sweden. Every advertisement for alcohol in Swedish newspapers has to dedicate an eighth of its area to a health message. And there are 11 warning messages to choose from, put forward by the Swedish public health institute.

While on the issue of labeling, it is amusing to see bottled water manufacturers faithfully declaring on the label zero values for all the nutrients. Since water is not a nutrient as per the definition, there is no need for pure water to be labeled highlighting its nutrient contents. But when it comes to alcohol more than the nutrition, the label has to convey in no uncertain terms the ill effects of consuming alcohol on human body. Though some believe moderate dose of alcohol is good for health, jury is still out on this claim and drinking being an addiction, it is better to be a teetotaler until such times conclusive proof is on the table regarding its absolute safety. As in the case of smoking, alcohol beverages must carry danger warning leaving the choice to the consumer as to how much risk one can take while taking alcohol.
